Referral Received
You submit by form, phone, or email. We confirm receipt and availability.
Eligibility Review
We review eligibility, waiver type, and the information provided.

Assessment & Tour
We complete an intake assessment and invite the team to tour the home.
Service Planning
We coordinate with the support coordinator to align with the ISP.
Welcome Home
We finalize move-in and begin services with a warm transition.
